Join the Consultants Register
Use this form to join the Consultants Register. The details you enter will appear in the Consultants Register Profiles.
You may join on-line, paying via credit card, or alternatively print out and mail in this form with your joining fee to the address below. If joining on-line, you will be notified by return e-mail within 24 hours that your registration has been received. If applying to join the Register by regular mail, please allow one week for confirmation. Whichever way you choose to join, once your registration has been processed you will receive notification by e-mail along with a request to confirm your chosen password. Once you receive a final e-mail indicating your confirmation has been noted, you can update your profile anytime by logging in to the Register using your password.
Access to your profile form for updating is password-protected. To access your profile, go to Change Profile, and enter your "User name" (your surname) and "Password" to log in. This will take you to your completed form again, and changing entries is a simple matter of deleting and retyping, then pressing "Submit".
Note: fields in red are mandatory.

In its viewable form, the "Previous Work" is a separate page, hyperlinked to the brief "Profile" section. It lists qualifications or experience gained, published works and relevant projects done over the previous 5 (?) years.
Will probably need a purpose-designed form to allow the applicant to create these two linked pages (their profile entry and their previous work page) from filling in one form.
