Port Pirie Video Conferencing Link
Introduction: On a monthly basis the Rural and Remote In-patient Multi Disciplinary and Nursing Service links with the Port Pirie Mental Health Team via the Video Conferencing network. Objectives: Review of current In-patients from Port Pirie area Discharge planning for current In-patients Follow up of recent discharges Discussion of other issues including the development of best practice models in continuity of care Attendance: Muli Disciplinary Team members of the In-patient Service Case Managers of current In-patients Case Managers of recently discharged clients Frequency: Second Friday of each month at 1330 for one hour Evaluation: As this is a trial the project will be evaluated after the first 4 months. |
